Find the forms here that you need to get signed for your Phys Ed class and/or for your Sports Team. 
Be sure to return the forms on time and to the correct person! 

For Sports Teams:

Want to play on a sports team at OTHS?  This is what you need to do!

 All OTHS Student Athletes [and your Parents/Guardians] must:

 Before tryouts:

1. Print off, read and sign the appropriate Sports Team Form and give it to your coach before trying out.
2. Print off, read and sign the Student Athlete Agreement Form and give it to your coach before trying out.  

After tryouts:

Emails will be sent to the parents/guardians of the student athletes who are successful in the tryouts.

1. Sign up for the School Cash Online System [under Quick Links on the OTHS Home Page] and then pay the appropriate Team Fee online.
